·  Address for pick up and drop off location.

·  Clients estimated weight and height.

·  Time and Date of your doctor's appointment

·  Estimated time for appointment and if appointment is a wait and return.

·  Are you in a wheelchair, need gurney transport or ambulatory.

·  Type of service: Door to Door, Curve to Curve

·  Do you need assistance from your bed to a wheelchair?

·  Do you have your own wheelchair or need one provided?

·  Do you have any specific conditions?

·  Any injuries or recent surgery?

·  Do you have your own oxygen cylinder, or do you need oxygen provided?

·  How many liters of oxygen are you prescribed by the doctor? (1,2,3 etc.)

·  Are there any stairs, how many stairs? are they straight-set or landing.

·  If a gurney is needed, are you able to sit up?

·  Is the hallway wide enough to maneuver the gurney? (Sometimes the gurney will

Fit into the room but cannot be maneuvered in the hallway)

·  Do you use a Standard or Bariatric wheelchair or gurney?

·  Is the nurse or family member riding along?

·  When was your last Covid-19 test?

We understand it seems to be a lot of information you must provide just for a trip to the doctor's office or hospital. However, all the information received from you will help us to allocate the necessary resources including personnel needed to perform the transportation in a comfortable and safe manner.
